Description of Services: Provides services that are individually tailored and flexible for people living with the effects of an acquired brain injury (ABI) within their own communities

Individual support sessions are scheduled on an hourly basis and are designed to support Clients based on their individual needs, working towards achievement of personal goals * the hours of support provided are subject to change over time, as determined by the individual’s needs, level of participation and the resources available * supports are offered one-on-one and/or in group settings, based on available resources to all Outreach Clients, and each Client’s strengths, goals, hopes and dreams * outreach services also include Justice Case Management and wait list management services provided to those in crisis or having short-term urgent needs * caregiver support groups and individual counseling emotional supports available or Clients and their caregivers/families
