Description of Services: Adult day program * social, recreational, and therapeutic activities for older adults in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
  • meals
  • individualized social and recreational plans for each client
  • caregiver support
  • blood pressure, weight and medical monitoring (by a registered nurse or registered practical nurse)
  • foot care (for a fee)
Requirements
Fees: $10 per day * additional fee for transportation ($5 one-way or $10 return trip)
Eligibility - Population(s) Served: Older adults, adults with disabilities, people with Alzheimer's Disease and related dementia's who need physical, social, or emotional supports * Local Health Integration Network care coordinators assess the needs of applicants
